  • Do we treat children?

    Yes!  Dr. Lewis treats ALL ages from the pediatric to adult population

  • What if you are a private pay patient or we do not accept your insurance?

    If we do not accept your insurance plan, Dr. Gideon J. Lewis will still provide high quality care with favorable and reasonable private pay pricing.


  • What insurance plans do we accept?

    We Accept MOST MAJOR insurance plans (i.e. Aetna, BCBS, Cigna, United, Medicare, Medicare Replacement).

  • What do I need to bring to my appointment?

    Insurance Card & Photo ID (i.e. Driver’s License)


    List of ALL current medications


    If your insurance requires a co-pay/deductible, please be prepared to pay that at the time of your visit. 



    Shoes most often worn including your running or athletic shoes. 


    In addition, it is helpful to write down a list of questions for Dr. Lewis so that he can ensure that he answers all of your questions in the exam room.

  • What can I expect as a New Patient?

    Our registration staff will ask you to fill out registration and health history forms. We ask that new patients arrive 15 mins before scheduled appointment time to complete your paperwork. These forms may be accessed on our website under New Patient Form.


  • Am I able to complete paperwork prior to my appointment?

    Yes. New patient paperwork is available on our website.



  • Do I need a referral to see Dr. Lewis?

    Our office only requires referrals for HMO Health Care Plans


  • Who can help me with a bill that I received?

    Our billing department is available to help you. For your convenience, please contact our Billing Department at 877-228-8218 for assistance.


  • Will my insurance cover the services?

    Each insurance plan is very different; therefore, we recommend that you contact your insurance plan to determine if the anticipated services are a benefit of your individual plan.


  • What is the typical amount of time spent at the office for new patient visit?

    1 hour to 1.5 hours (from checkin to checkout)



  • Does Dr. Lewis ONLY practice Sports Medicine?

    No, Dr. Gideon J. Lewis offers the highest quality in comprehensive foot and ankle care. He gladly accepts a wide range of patients including pediatric, and adult population.
